Multidisciplinary Care of the Urothelial Cancer Patient

Invasive urothelial cancer is a heterogeneous disease with a clear need to identify better methods to differentiate those patients who can be managed less aggressively from those who require aggressive multimodal therapy. Novel diagnostic tools, new treatments and methods for treatment delivery, modern surgical techniques, and a better understanding of the molecular underpinnings that drive the disease will assuredly improve treatment selection and lead to improved outcomes for patients.


As Guest Editors for this issue of Urologic Clinics of North America , we aimed to recruit thought leaders in urologic oncology, radiation oncology, and medical oncology as well as translational scientists who focus on invasive urothelial cancer to provide a comprehensive overview of the multidisciplinary care of the urothelial cancer patient. The issue begins with novel diagnostic techniques as well as new treatments and treatment delivery methods in non-muscle-invasive high-grade disease and then shifts toward the management of muscle-invasive disease with in-depth discussions focused on trimodality bladder preservation, and a thorough discussion of the conflict between the data supporting and the reality of neoadjuvant chemotherapy utilization. Also discussed is the current status of novel biomarkers to predict outcomes for localized disease after primary treatment. We next focus on the role for less aggressive surgery in muscle-invasive disease, followed by an in-depth commentary on the evolution of modern surgical techniques including robotic surgery and its clinical impact as compared with cost. The recently reported comprehensive molecular characterization of invasive urothelial cancer by The Cancer Genome Atlas project provided a new framework for the development of novel therapies, and in this issue, we also review the impact of these findings on the management of localized urothelial cancer as well as metastatic disease. We are delighted that this group of thought leaders agreed to work on this issue and we are confident that the reader will walk away with a comprehensive and up-to-date understanding of multimodal therapy in invasive urothelial cancer.
