Haemostasis for Nesbit’s procedure
The Nesbit’s procedure for Peyronie’s is a common procedure performed by most urologists. However, on occasion all surgeons can run into difficulties, especially when trying to achieve haemostasis during and after the operation. If a large haematoma develops postsurgically then always evacuate this as subsequently patients will develop contracture of the penis and in some instances chronic pain which can be quite debilitating.
I always perform surgery through a circumferential incision, and in most cases will circumcise the patient. With all circumcisions there is a tendency to bleed, therefore ensure that you have sutured the frenular area as if you just use diathermy alone in these cases then subsequently on performing artificial erection, bleeding can be triggered from these diathermied areas by the saline infusion. I tend to reconstruct the frenular area with Vicryl.
