Diverticula of Esophagus (Zenker’s)
DEFINITION:
Zenker’s Diverticula: upper esophageal diverticula
Traction Diverticula: midesophageal diverticula
Epiphrenic Diverticula: distal esophageal diverticula
PATHOPHYSIOLOGY:
Zenker’s: Impaired cricopharyngeal compliance, usually due to fibrotic changes, causes increased intrabolus pressure with swallowing
Relaxation of UES is usually normal
The result is increased hypopharyngeal pressure with herniation at a weak point just above the cricopharyngeus
Traction: thought to result from esophageal motility disorders
CLINICAL MANIFESTATIONS/PHYSICAL EXAM:
Zenker’s: regurgitation of undigested food, bad breath, visible lump on side of neck, dysphagia in lower neck area
DIAGNOSTIC STUDIES:
Barium Swallow
EGD
