Diverticula of Esophagus (Zenker’s)





DEFINITION:



  • Zenker’s Diverticula: upper esophageal diverticula


  • Traction Diverticula: midesophageal diverticula


  • Epiphrenic Diverticula: distal esophageal diverticula


PATHOPHYSIOLOGY:



  • Zenker’s: Impaired cricopharyngeal compliance, usually due to fibrotic changes, causes increased intrabolus pressure with swallowing



    • Relaxation of UES is usually normal


    • The result is increased hypopharyngeal pressure with herniation at a weak point just above the cricopharyngeus


  • Traction: thought to result from esophageal motility disorders


CLINICAL MANIFESTATIONS/PHYSICAL EXAM:



  • Zenker’s: regurgitation of undigested food, bad breath, visible lump on side of neck, dysphagia in lower neck area


DIAGNOSTIC STUDIES:



  • Barium Swallow


  • EGD
