Abdominal Pain: Chronic





DEFINITION:



  • Pain that has remained unchanged for months or years can be safely classified as ‘chronic’



    • Divided into diagnosable (intermittent or constant) and undiagnosable


  • Chronic undiagnosed abdominal pain: pain that is present for at least 6 months without diagnosis despite appropriate evaluation



    • ♀ > ♂, A history of sexual or physical abuse is common; Pain is described in vague peculiar terms; Multiple somatic complaints

Chronic Intermittent Pain:



















Mechanical:


Intermittent intestinal obstruction (hernia, intussusception, adhesions, volvulus), Gallstones, Ampullary stenosis


Inflammatory:


IBD, Endometriosis/Endometritis, Acute relapsing pancreatitis, Familial Mediterranean Fever


Neurologic and Metabolic


Porphyria, Abdominal epilepsy, Diabetic radiculopathy, Nerve root compression or entrapment, Uremia


Miscellaneous:


IBS, Nonulcer dyspepsia, Chronic mesenteric ischemia, Mittelschmerz
